Transition Support Worker

LifeNow is just about as pleased as you could possibly get, to announce the search for our first “Transition Support Worker” in partnership with Derian House Children’s Hospice.

This is a fabulous milestone in our development and we are looking forward to working together with Derian House on this new project.

What’s it all about?

Some time ago we started with a new vision in two parts. The first to develop nursing support and the second, to develop family support. Our search for a lead nurse continues but when our friends at Derian hear about our proposed work with young adults and their families, it got them thinking that we needed to talk.

Derian House in Chorley Lancashire is a very well established and respected charity that supports children into young adulthood to the age of 25 years. After that, young adults must get support from the general adult services and hospices. The transition into this new phase of life is rarely straightforward and to cut a long story short over various cups of tea, we agreed to work together to pioneer a new role. This will be to support young adults 24-28 years old to establish themselves more securely in the adult world and with independence where that is appropriate and to support families during this period.

We believe this project will open a new path for us at LifeNoW and it may become a model of support that we adopt with other Hospice partners in time. First of all, we need to do some experimenting and learning in this project over the coming 12 months. Derian House will be the employer and LifeNow is providing the grant funding to make it all possible.

£20,100-£23,650 dependent on experience

Full time 12 month fixed term contract

LifeNoW Transition Support Worker 

LifeNoW is working in partnership with Derian House to provide this exciting new opportunity. The post holder will work alongside the team at Derian House Children’s Hospice with an identified group of young adults aged 24-28 years to facilitate the transition from support networks provided by the Hospice following discharge from these services.

The main purpose of this post is to work with young adults with a life-limiting condition to offer guidance and support for transition from children’s hospice services, to raise aspirations and help remove barriers and also to develop transition pathways and services which will promote autonomy and independence.

The post is based at Derian House Children’s Hospice in Chorley and will work as part of the multi-disciplinary team within the Hospice. For further details about these positions or to arrange an informal discussion please contact a member of staff from the Senior Care Team on 01257 233300.
