Life NoW Working in Partnership
We ensure people get our support by working in partnership with others.

Our first project which is live, is the provision of a Support Worker at Derian House Children’s Hospice in Chorley, Lancashire. Here we fulfil an objective to help young adults who would otherwise no longer receive support, growing too old for the children’s hospice and its outreach services. The role assists young people to build supportive social networks in the outside world and ensures they have as much support as possible from agencies.

We are helping young adults to manage complex health conditions by providing advice, support and the coordination necessary to remain at home and live well.

We plan to fund other similar arrangements with hospices as we learn from this first project that will focus on some 20 young people and their families in 2019.

We would love to hear from other hospices and individuals who would like to participate in this developing work which in time may fund more support workers and nurses in partnership with children’s and adult hospices.
