How We Work

We create LifeNoW Support Workers in partnership with hospices

Our first project which is live, is the provision of two Support Workers at Derian House Children’s Hospice in Chorley, Lancashire.

Here we fulfil an objective to help young adults who would otherwise no longer receive support, growing too old for the children’s hospice and its outreach services.

The roles assist young people to build supportive social networks in the outside world and ensures they have as much support as possible from agencies.

Our second post is a nursing role which brings valuable expertise and experience.

We are helping people in their 20s & 30s to manage complex health conditions by providing advice, support and the coordination necessary to remain at home and live well.

We have extended our funding commitment with Derian House for another two years to 2024 and we continue to raise the funds to provide more LifeNoW workers in new settings across the North West and North Wales.

We would love to hear from other hospices and individuals who would like to participate in this developing work.

Join us in being part of something great!

We can make a difference to young adults and their families with your help!
